Abreakce – úspěch i problém v psychoterapii neuróz

V naší kazuistické studii demonstrujeme abreakci s katarzí u pacientky s histriónskou poruchou osobnosti a současně i neurotickými, dissociativními (konverzními) poruchami. Etiologie vykazovala typická traumata a symptomatologie odpovídala známým hysterickým projevům. Cenný diagnostický materiál přinesla analýza dopisu pacientky, její kresby a snu a finální interpretace konverzních symptomů. K samotné abreakci došlo po analýze snu, a to spontánně, písemnou formou a po jejím psychoterapeutickém zpracování se dostavil zdánlivě celkový léčebný úspěch. Náhodně získaná katamnestická data však potvrdila, že se trvalejší efekt týkal jen symptomatických poruch a že kompenzace strukturální poruchy byla jen dočasná. Navíc byla navozena jinými faktory, a proto se znovu dekompenzovala v nové frustrační situaci. Šlo o hrozící pracovní a společenský debakl, který pacientka řešila účelovou pseudoregresivní inscenací hypochondrického charakteru. Cílem byla redukce úzkosti ze sociálního odmítnutí (získáním statusu nemocné) a pocitu méněcennosti (ziskem renty). Přitom došlo ke změně inscenace poruch psychosomatického (konverzního) typu na psychopatologické (dissociativní).

Summary

In our casuistic study we demonstrate abreaction with catharsis in a patient suffering from histrionic personality disorder and also from neurotic, dissociative (conversion) disorders. Etiology showed typical traumas and symptomatology corresponded to well-known hysterical manifestations. Valuable diagnostic material was obtained via analysis of the patient's letter, her drawing and a dream and also via final interpretation of conversion symptoms. Abreaction itself occurred after the dream was analysed. It manifested spontaneously in written form and a seemingly overall therapeutic success was achieved after it was psychotherapeutically processed. Accidentally obtained catamnestic data, however, confirmed that a more lasting effect applied only to symptomatic disorders and that the compensation of the structural disorder was only temporary. Furthermore, this compensation was induced by other factors and decompensation therefore occurred in a new frustrating situation. This situation concerned an imminent social and work-related debacle, which the patient tried to solve by purposeful pseudoregressive staging of hypochondriacal nature. The goal was to reduce anxiety of social rejection (by acquiring the status of an ill person) and of the sense of inferiority (by acquiring a rent benefit). At the same time the staging of disorders changed from the psychosomatic (conversion) types to psychopathologic (dissociative) types.
