Autoimunitná encefalitída asociovaná s protilátkami proti glutamátovým receptorom (typ NMDA) - prípad z praxe práca

Súhrn

Limbická encefalitída je zápalové ochorenie mozgu s akútnym až subakútnym rozvojom. U viac ako 2/3 pacientov dominuje psychiatrická symptomatika, v popredí klinického obrazu dominuje úzkosť, depresia, zmena osobnosti, poruchy vnímania v zmysle halucinácií, bludná produkcia, dezorganizované myslenie a konanie, agitovanosť, poruchy spánku a osobnosti, katatónne príznaky. Autoimunitná encefalitída je asociovaná s protilátkami proti glutamátovým receptorom (typ NMDA), glutamátovým receptorom (typ AMPA), GABAB receptorom, VGKC – asociovaným proteínom (LGI 1, CASPR2, TAG-1/contactin 2) a napäťovo riadeným káliovým kanálom (VGKC). Pri pozitívnom laboratórnom náleze je nutné pátrať po prítomnosti nádorov. V posledných rokoch prekonala táto oblasť neuroimunológie prudký rozvoj. Autori predstavujú prípad pacientky, kde pozitivita protilátok proti NMDA receptorom špecifikovala ochorenie ako autoimunitnú (limbickú) encefalitídu s protilátkami proti NMDA receptorom pri teratóme ovária. V poslednom období nie je diagnostika tohto ochorenia ojedinelá a preto je v rámci diferenciálnej diagnostiky prvej epizódy psychózy nutné myslieť aj na tento typ ochorenia mozgu.

Summary

Lymphatic encephalitis is an inflammatory disease of the brain with acute or subacute development. More than 2/3 of patients have psychiatric symptoms. In clinical picture dominates anxiety, depression, personality changes, perceptual disorders as hallucinations, delusional production, disorganized thinking and acting, agitation, sleep and personality disorders, catatonic symptoms. Autoimmune encephalitis is associated with antiglutamate receptor antibodies (NMDA type), glutamate receptor (type AMPA), GABAB receptor, VGKC-associated protein (LGI1, CASPR2, TAG-1 / contactin 2) and a voltage controlled potassium channel (VGKC). If laboratory findings are positive further investigation of the presence of tumors must be done. There was rapid development in this area of neuroimmunology in last couple of years. The authors present the patient case report, where the positivity of antibodies against the NMDA receptor, specified the disease as autoimmune (limbic) encephalitis with antibodies against NMDA receptors in the teratoma of the ovary. Recently, the diagnosis of this disease is not unusual and therefore it is essential, within differential diagnostics of the first episode of psychosis, to think of this type of brain disease.
